This commentary highlights the key points in the recent article by Blum et al. [1] that advocates for medical practices to adapt conservation measures to reduce waste and lower their carbon footprints. Healthcare in the United States produces considerable amounts of pollutants into the air, water, and soil. It is both a significant challenge and an opportunity for healthcare to reduce its carbon footprint. Medical offices, surgical centers and hospitals are responsible for 10% of chemicals that produce smog and 10% of greenhouse gasses [2]. Estimates show that the amount of greenhouse gasses and smog production from healthcare, among other factors, contributes to 405,000 years of disability-adjusted lives lost annually in the United States [2].

Examining BRCA mutations in preoperative breast cancer patients is very important when selecting a surgical procedure. Although there are advantages and disadvantages associated with knowing about the presence of genetic mutations, including for the patient’s family, there are many benefits for the patient. BRCA genetic testing should be recommended for patients who are strongly suspected of being positive for a BRCA mutation. 

Epigenetics is the study of heritable modifications to gene expression, such as DNA methylation, histone modifications, and RNA modifications, that do not alter the nucleotide sequence of the corresponding gene. Recently, RNA modification has emerged as a novel research focus. We have detected over 170 different RNA modifications,  such as N6-methyladenosine (m6A), N7-methylguanosine (m7G), 5-methylcytosine (m5C), N1-methyladenosine (m1A), N3-methylcytosine (m3C), and pseudouridine (ψ).These modifications are important for biological processes like RNA metabolism and post-transcriptional regulation. 

In the early phases of the pandemic caused by the SARS-CoV-2 virus (severe acute respiratory syndrome coronavirus 2) the emphasis of diagnosis and treatment was on acute respiratory distress syndrome (ARDS).

Proliferative glomerulonephritis with monoclonal immunoglobulin deposits (PGNMID) was first described by Nasr et al. in 2004 with a subsequent study composed of a larger patient cohort a few years later [1,2]. Although more commonly occurring in older adults, PGNMID has since been reported in a wide range of age groups including children [3-5].  PGNMID is categorized as a monoclonal gammopathy of renal significance (MGRS), and is additionally included in the recently expanded concept of monoclonal gammopathy of clinical significance (MGCS), which encompasses disorders of any organ system related to underlying plasma cell or B-cell clones [6]; however, most patients with PGNMID do not show evidence of a circulating monoclonal protein or clonal plasma cell proliferation, and monoclonal gammopathy has not been reported in any pediatric patients with the disease. Herein we briefly discuss the clinical and histopathologic features of PGNMID, as well as advances in our understanding of the pathogenesis and clinical course of the disease.
